How to raise an issue.

Creating an issue
- Open a project → Issues.
- Select Add Issue.
- Enter a title, description and impact.
- Set the impact level (High, Medium, Low).
- Add an owner and the action/decision to resolve it.
Issues are risks that have materialised — they need active resolution.
